GDPR/Privacy Notices

If you would like to request copies of your medical records, you will need to complete a Subject Access Request form and bring in proof of ID. These forms are available at all sites and online in the box below. Please note that we will only supply copies of data we as a surgery are the Data Controller of. If you want access to data held by another organisation, such as a community health service or hospital, you will need to apply to them directly. 

We have 30 days to furnish you with this information, which starts the date we are in receipt of your ID documents. We outsource our Subject Access Requests to a company called iGPR - they will contact you once your records are available, so please ensure an email address is provided in your request. Please do not contact them until you have submitted your request to us directly. 

If you are simply wanting to view your records, please contact your local surgery and ask to speak with the Team Leader to book an appointment with them. You will need to bring photographic ID to your appointment. You may also find the information you need through the NHS App, which we would recommend checking first.
